To respond to original post, the problems with PUG are two: lack of patience among members and leadership confusion. For the former, you see people want to rush through mission by running pass minions that simply cannot be bypassed. Or people simply quit the team after a death or two. For example in desert PUG often disbanded short of reaching next town, whereas with henchies or guildmates with perserverance you often can make it. Leadership confusion comes NOT because lack of, but because too many people want to assume the role and can't compromise. That again, can be due to lack of patience, because people want to get to their objective FAST and don't want to take time to iron out their difference. Ill-planned rush is THE single cause of many PUG mission failure. Yesterday I helped my wife completing Elona mission with henchies in just two tries, while she (a monk) had spent three days with countless PUG's who, for some reason, preferred to run into middle to be slaughtered by different groups of minotaur or enchanted sword patrols than taking those key ones out one by one.
